INSTRUCTIONS
1. Sign and fill out the 1st spot completely on the top of the 2nd page of the petition.
2. This includes your signature, print your name clearly, your address & #, by municipality write your towns name, put a check mark in the box and finally date it.
3. Take the petition to other registered voters like your spouse, other family members, neighbors, and friends and personally witness each of them sign this petition.
4. Make sure they fill out their section on the page completely in front of you.
5. When you have finished gathering signatures from people you know, bring the petition to Your Bank or the Library to get it Notarized it’s FREE.
6. You will need to sign the bottom of page 2 where it states Signature of Circulator in front of the notary at that time and fill out (your name), (your address) and (date) to (date) info.
7. Mail back the two page petition form to:
The Committee to Recall that mailed it to you.
